In Their Habitat (Co-design)
This project is a small co-design project with young people (15 and older) with disability and their parents/carers. This is designed to support and inform a larger 2-year proof of concept project ‘In Their Habitat’ that has been co-funded by the Research Foundation of the Cerebral Palsy Alliance and the Sydney Children’s Hospital Foundation. The goal of ‘In Their Habitat’ is to develop a consistent approach to paediatric rehabilitation where assessment of movement will not be made in the rarefied clinical environment but in the community with children undertaking the activities that make up their daily life.

mHealth app and electronic medical record systems
Digital health technologies can enable more efficient and collaborative healthcare for both patients and practitioners as part of broader digital reforms designed to deliver better care and improved health outcomes. The research team is investigating the integration of clinically relevant mobile health app data into electronic medical record systems.

Cancer Shared Care Program
This project aims to establish and evaluate a web-based interactive cancer shared care plan that supports long term integrated care of cancer patients for colorectal and breast cancers. Also, to evaluate a GPCanShare Assessment for patients with lung cancer. Current models of cancer follow-up care involve either specialist services continuing to provide routine care for long periods or transfer of care to general practice with little or no specialist involvement. Our previous research demonstrated that a shared model of care is acceptable to patients and providers.

Evaluation of a virtual health hub for people at risk of homelessness
This project supports the evaluation of the first pilot location of the NSW Virtual Health Hub Model of Care which is providing telehealth solutions to promote accessible and comprehensive healthcare services for people at risk of homelessness. Individuals experiencing homelessness and residing in boarding houses in the Sydney metropolitan region face a range of challenges in accessing and receiving necessary medical services.

SHAReD: Shared Health Arrangements Research & Development
The SHAReD study aims to improve the coordination of care between mental health services and primary care to better manage risk factors for heart disease and diabetes and improve the health of people living with severe mental illness. It will introduce telehealth and web-based shared care plans between consumers, mental health services and general practices allowing them to better communicate with each other and plan care.

Development of Mental Health Shared Care e-plans
The aim of this project is to evaluate the feasibility of an online shared care plan supporting communication for clozapine treatment, preventive care and behavioural change in the long-term shared management of people with a lived experience of severe mental illness. The care plan has been developed to support communication between general practitioners, specialist services, consumers, carers and other care team members. This intervention is being implemented with the My GP & Me shared care program running in the South Eastern Sydney Local Health District with the Eastern Suburbs Mental Health Service.
