Innovative models of care and cost effectiveness of community aged care and rehabilitation services, including telehealth services, integrated health service, outpatient models of care for individuals following requiring ongoing rehabilitation; and geriatric outreach services into residential aged care facilities (RACFs) and the community to minimise or eliminate unnecessary presentations to Emergency Departments and facilitate early transfer back to RACFs.
