The Youth Advocate Program (YAP) is a unique person-centred program, provided by Life Without Barriers (LWB), which aims to achieve successful transition to independent, supported living or to birth family for vulnerable and at-risk young people. 

The approach connects the young person leaving out of home care or the juvenile justice system in Australia with 'Youth Advocates'. The 'Youth Advocates' work with the individual for up to 15 hours a week for six to nine months, assisting their transition and supporting them to achieve other goals associated with their transition. 

The Social Policy Research Centre evaluated the results of this program to assess its effectiveness in achieving objectives. Also, to inform the ongoing outcome measures needed to ensure the program can be effectively evaluated in the future and to determine benefits to a broader cohort of young people outside of LWB.
